Background Check Information

In our commitment to ensure the safety of our families, volunteers, and staff, all applicants 18 years and older are required to complete a background check. This information will be kept confidential and used only for the purpose of conducting a background check. Application verification and release

I recognize the TEDCenter is relying on the accuracy of the information contained herein. Therefore, I attest and affirm that all the information I’ve provided is true and correct. I authorize the TEDCenter to contact any person or entity listed in this application and I further authorize any such person or entity to provide the TEDCenter information or opinions relating to my background or qualifications. I voluntarily release the TEDCenter and any person or entity listed herein from liability involving the communication of information related to my background or qualifications. I authorize the TEDCenter to conduct a criminal background check with this volunteer application. If my volunteer application is approved, I further authorize the TEDCenter to conduct additional background checks as-needed basis until I request to cease my volunteer services. I understand that the TEDCenter has the right to accept or reject my application for any reason. If I am approved, I agree to abide by all policies and procedures for the protection of the TEDCenter and all who visit. All information contained in this application and collected from background checks is considered highly sensitive and confidential.
